Live Scan Guide: How To Get Your RAP Sheet
Follow the instructions below if you DO NOT qualify for a fee waiver.

2. Download the Live Scan Form below
You can fill in the form in two ways:
- Digitally - type into the PDF fields, then print & sign it.
- Print first - fill it all in with a pen.
If you don’t have access to a printer, go to your local library or contact your college's Rising Scholars program coordinator.
3. Filling in the form
Fill out the rest of your personal info in the “Applicant Information” section. Make sure that your address is filled in correctly! The DOJ will mail you a copy of your RAP sheet.
To ensure that The Access Project gets a copy too, we pre-filled our address in the designee slot at the bottom of the form.
This means that a copy of your RAP sheet will be mailed directly to us. You will not be eligible for the program until we receive your RAP sheet.
4. Visit your nearest Live Scan location
Bring your form and an ID!
Fees and hours will vary from site to site. Some sites require appointments, other require walk-ins. Call the site beforehand to learn about hours of operations and costs.
Talk to your college’s Rising Scholars program manager to see if you can get a reimbursement for any costs. TAP is not responsible for any payments.
You must bring a driver’s license or other form of ID with you. Acceptable forms of ID can be found here.
After your Live Scan is complete and the operator fills out the bottom part of the form, take a photo of the form (think of this as your “receipt”).
Wait to receive your RAP sheet. Then send us a copy.
- Wait to receive the RAP sheet in the mail
- Upon receiving your RAP sheet in the mail, upload it
- RAP Upload Form
- Take a scan. It must be CLEAR and LEGIBLE

- After the upload, you will be redirected to our official registration page
- No RAP sheet = No participation in the program
